Cost-effectiveness of severe acute respiratory coronavirus virus 2 (SARS-CoV-2) testing and isolation strategies in nursing homes

Objective:
Nursing home residents may be particularly vulnerable to coronavirus disease 2019 (COVID-19). Therefore, a question is when and how often nursing homes should test staff for COVID-19 and how this may change as severe acute respiratory coronavirus virus 2 (SARS-CoV-2) evolves.
Design:We developed an agent-based model representing a typical nursing home, COVID-19 spread, and its health and economic outcomes to determine the clinical and economic value of various screening and isolation strategies and how it may change under various circumstances.
Results:Under winter 2023–2024 SARS-CoV-2 omicron variant conditions, symptom-based antigen testing averted 4.5 COVID-19 cases compared to no testing, saving $191 in direct medical costs. Testing implementation costs far outweighed these savings, resulting in net costs of $990 from the Centers for Medicare & Medicaid Services perspective, $1,545 from the third-party payer perspective, and $57,155 from the societal perspective. Testing did not return sufficient positive health effects to make it cost-effective [$50,000 per quality-adjusted life-year (QALY) threshold], but it exceeded this threshold in ≥59% of simulation trials. Testing remained cost-ineffective when routinely testing staff and varying face mask compliance, vaccine efficacy, and booster coverage. However, all antigen testing strategies became cost-effective (≤$31,906 per QALY) or cost saving (saving ≤$18,372) when the severe outcome risk was ≥3 times higher than that of current omicron variants.
Conclusions:SARS-CoV-2 testing costs outweighed benefits under winter 2023–2024 conditions; however, testing became cost-effective with increasingly severe clinical outcomes. Cost-effectiveness can change as the epidemic evolves because it depends on clinical severity and other intervention use. Thus, nursing home administrators and policy makers should monitor and evaluate viral virulence and other interventions over time.
A multi-country comparison of jurisdictions with and without mandatory nutrition labelling policies in restaurants: analysis of behaviours associated with menu labelling in the 2019 International Food Policy Study

Objective:
To examine differences in noticing and use of nutrition information comparing jurisdictions with and without mandatory menu labelling policies and examine differences among sociodemographic groups.
Design:Cross-sectional data from the International Food Policy Study (IFPS) online survey.
Setting:IFPS participants from Australia, Canada, Mexico, United Kingdom and USA in 2019.
Participants:Adults aged 18–99; n 19 393.
Results:Participants in jurisdictions with mandatory policies were significantly more likely to notice and use nutrition information, order something different, eat less of their order and change restaurants compared to jurisdictions without policies. For noticed nutrition information, the differences between policy groups were greatest comparing older to younger age groups and comparing high education (difference of 10·7 %, 95 % CI 8·9, 12·6) to low education (difference of 4·1 %, 95 % CI 1·8, 6·3). For used nutrition information, differences were greatest comparing high education (difference of 4·9 %, 95 % CI 3·5, 6·4) to low education (difference of 1·8 %, 95 % CI 0·2, 3·5). Mandatory labelling was associated with an increase in ordering something different among the majority ethnicity group and a decrease among the minority ethnicity group. For changed restaurant visited, differences were greater for medium and high education compared to low education, and differences were greater for higher compared to lower income adequacy.
Conclusions:Participants living in jurisdictions with mandatory nutrition information in restaurants were more likely to report noticing and using nutrition information, as well as greater efforts to modify their consumption. However, the magnitudes of these differences were relatively small.

The Australian SKA Pathfinder (ASKAP) radio telescope has carried out a survey of the entire Southern Sky at 887.5 MHz. The wide area, high angular resolution, and broad bandwidth provided by the low-band Rapid ASKAP Continuum Survey (RACS-low) allow the production of a next-generation rotation measure (RM) grid across the entire Southern Sky. Here we introduce this project as Spectral and Polarisation in Cutouts of Extragalactic sources from RACS (SPICE-RACS). In our first data release, we image 30 RACS-low fields in Stokes I, Q, U at 25$^{\prime\prime}$ angular resolution, across 744–1032 MHz with 1 MHz spectral resolution. Using a bespoke, highly parallelised, software pipeline we are able to rapidly process wide-area spectro-polarimetric ASKAP observations. Notably, we use ‘postage stamp’ cutouts to assess the polarisation properties of 105912 radio components detected in total intensity. We find that our Stokes Q and U images have an rms noise of $\sim$80 $\unicode{x03BC}$Jy PSF$^{-1}$, and our correction for instrumental polarisation leakage allows us to characterise components with $\gtrsim$1% polarisation fraction over most of the field of view. We produce a broadband polarised radio component catalogue that contains 5818 RM measurements over an area of $\sim$1300 deg$^{2}$ with an average error in RM of $1.6^{+1.1}_{-1.0}$ rad m$^{-2}$, and an average linear polarisation fraction $3.4^{+3.0}_{-1.6}$ %. We determine this subset of components using the conditions that the polarised signal-to-noise ratio is $>$8, the polarisation fraction is above our estimated polarised leakage, and the Stokes I spectrum has a reliable model. Our catalogue provides an areal density of $4\pm2$ RMs deg$^{-2}$; an increase of $\sim$4 times over the previous state-of-the-art (Taylor, Stil, Sunstrum 2009, ApJ, 702, 1230). Meaning that, having used just 3% of the RACS-low sky area, we have produced the 3rd largest RM catalogue to date. This catalogue has broad applications for studying astrophysical magnetic fields; notably revealing remarkable structure in the Galactic RM sky. We will explore this Galactic structure in a follow-up paper. We will also apply the techniques described here to produce an all-Southern-sky RM catalogue from RACS observations. Finally, we make our catalogue, spectra, images, and processing pipeline publicly available.

Background: In the treatment of bloodstream infections, the identification of the causal pathogen, and the evaluation of its susceptibility to antibiotics, often serve as the rate-limiting steps of the patient’s hospital stay. The GenMark Dx ePlex blood culture identification gram-positive (BCID-GP) panel aims to alleviate this bottleneck, thereby reducing the risk of severe complications and the spread of resistance, using electrowetting technology to detect the most common causes of GP bacteremia (20 targets) and 4 antimicrobial resistance (AMR) genes. We hypothesized that implementation of the ePlex BCID-GP panel would improve antimicrobial choice and de-escalation where appropriate. Methods: A mixed blinded and unblinded study was conducted to assess the effect of the BCID-GP panel on the outcomes and antibiotic stewardship of GP bacteremic patients before ePlex results were made clinically available (before implementation, N = 73) and once they accompanied the standard-of-care work-up (after implementation, N = 82). Differences in time to different benchmarks between the 2 modalities and the effect on patient outcomes were analyzed using null-hypothesis significance testing. Results: During the study, the BCID-GP panel identified 63 (42%) Staphylococcus epidermidis isolates, 31 (21%) Staphylococcus spp, 24 (16%) Staphylococcus aureus isolates, 12 (8%) Streptococcus spp, and 7 (5%) Enterococcus spp, and results were similar in the pre- and postimplementation groups (P = .13). The panel saved an average of 32.0 ± 24.2 hours in pathogen identification over standard-of-care methods, with no statistical difference made by the clinical availability of the data (Table 1). In terms of susceptibility testing, the panel saved an average of 70.1 ± 58.2 hours but with less unity between the 2 cohorts (P = .005). Of the 66 cases with follow-up, identification via ePlex indicated an escalation of therapy in 20 (30%) and a narrowing of coverage in 31 (47%). In patients identified to have Staphylococcus aureus, BCID-GP could change antimicrobial therapy in 79%; the need for escalation of antibiotics was identified in 58% of cases. In patients with Staphylococcus epidermidis bacteremia, implementation of BCID-GP panel could have resulted in de-escalation of antimicrobial therapy in 67% of patients. The implementation of the BCID-GP panel was correlated with no significant change of in-hospital mortality (P = .72) but was correlated with a significantly decreased death-censored total length of stay (LOS) (P < .001) and LOS after culture (P = .001). Conclusions: Our study has demonstrated that nonculture identification of bacteria and susceptibility can result in major improvements in antimicrobial therapy in patients, particularly those with contaminants identified.

The rotorcraft is a complex dynamical system that demands specialist modelling skills, and a high level of understanding of the aeromechanics arising from the main rotor wake and aerodynamic couplings. One such example is the difficulty predicting off-axis responses, particularly in hover and low-speed flight, associated with induced velocity variation through the rotor disk resulting from the rotor wake distortions. Various approaches have been developed to deal with this phenomenon but usually demand prerequisites of high levels of expertise and profound aerodynamic knowledge. This paper presents a new and practical approach to capturing this wake distortion through an augmented rotor inflow model. The proposed model is coupled with a nonlinear simulation using the FLIGHTLAB environment, and comparisons are made between the simulation results and flight test data from the National Research Council of Canada’s Advanced System Research Aircraft in hover and low speed. Results show good predictability of the proposed nonlinear model structure, demonstrated by its capability to closely match the time responses to multi-step control inputs from flight test. The results reported are part of ongoing research at Liverpool and Cranfield University into rotorcraft simulation fidelity.

The research reported in this paper is aimed at the development of a metric to quantify and predict the extent of pilot control compensation required to fly a wide range of mission task elements. To do this, the utility of a range of time- and frequency-domain measures to examine pilot control activity whilst flying hover/low-speed and forward flight tasks are explored. The tasks were performed by two test pilots using both the National Research Council (Canada)’s Bell 412 Advanced Systems Research Aircraft and the University of Liverpool’s HELIFLIGHT-R simulator. Handling qualities ratings were awarded for each of the tasks and compared with a newly developed weighted adaptive control compensation metric based on discrete pilot inputs, showing good correlation. Moreover, in combination with a time-varying frequency-domain exposure, the proposed metric is shown to be useful for understanding the relationship between the pilot’s subjective assessment, measured control activity and task performance. By collating the results from the subjective and objective metrics for a range of different mission task elements, compensation boundaries are proposed to predict and verify the subjective assessments from the Cooper-Harper Handling Qualities Rating scale.

We describe an ultra-wide-bandwidth, low-frequency receiver recently installed on the Parkes radio telescope. The receiver system provides continuous frequency coverage from 704 to 4032 MHz. For much of the band ( ${\sim}60\%$ ), the system temperature is approximately 22 K and the receiver system remains in a linear regime even in the presence of strong mobile phone transmissions. We discuss the scientific and technical aspects of the new receiver, including its astronomical objectives, as well as the feed, receiver, digitiser, and signal processor design. We describe the pipeline routines that form the archive-ready data products and how those data files can be accessed from the archives. The system performance is quantified, including the system noise and linearity, beam shape, antenna efficiency, polarisation calibration, and timing stability.

Summary
Introduction
The perspectives of older people on dignity in care have been largely overlooked in British policies, and the tendency has been to look at ways in which care providers should ‘deliver dignity’ by reference to agreed standards. For example, the Dignity Challenge developed in 2006 identified 10 dignity tests against which services could be evaluated (Cass et al, 2009). In this project, the research team took the view that a better understanding of dignity in later life was needed, which was informed by the experiences of older people, including, but not limited to, those with experiences as service users.
Four questions shaped the research:
1. What preparations do older adults who are facing death at a near but uncertain time make for the process of dying and death?
2. What accounts are given about dignity in daily life by older adults during this phase in their life?
3. What resources are available to these older adults to draw on?
4. What factors are perceived to support or undermine a sense of dignity?
Dignity has both personal and social meanings, and the two are closely interrelated. The theoretical model developed by Nordenfelt (2004) was a valuable starting point for this project, as it takes both into account. It proposes four ‘varieties’ of dignity. First, the dignity of merit refers to the high rank of individuals because of the social role they occupy (the historical meaning of dignity) as well as to dignity earned through socially valued achievements. The dignity of moral stature concerns individual conduct and is linked to self-respect, which is shaped by prevalent cultural values, giving it a social as well as personal meaning. Importantly, this type of dignity also draws attention to the conduct of staff in care services and the impact of this on their dignity and moral stature. The third variety – the dignity of identity – is as Nordenfelt expressed it, ‘the dignity that we attach to ourselves as integrated and autonomous persons’ (2009, p 33). It is evidently challenged in old age by failing health and the loss of capacity for self-care. The dignity of identity also has social as well as personal meaning, exemplified in the institutionalisation of older people and the associated loss of individual identity. The fourth of Nordenfelt's varieties is the dignity of being human.
Low-fat yogurt consumption reduces biomarkers of chronic inflammation and inhibits markers of endotoxin exposure in healthy premenopausal women: a randomised controlled trial

The anti-inflammatory mechanisms of low-fat dairy product consumption are largely unknown. The objective of this study was to determine whether low-fat yogurt reduces biomarkers of chronic inflammation and endotoxin exposure in women. Premenopausal women (BMI 18·5–27 and 30–40 kg/m2) were randomised to consume 339 g of low-fat yogurt (yogurt non-obese (YN); yogurt obese (YO)) or 324 g of soya pudding (control non-obese; control obese (CO)) daily for 9 weeks (n 30/group). Fasting blood samples were analysed for IL-6, TNF-α/soluble TNF II (sTNF-RII), high-sensitivity C-reactive protein, 2-arachidonoyl glycerol, anandamide, monocyte gene expression, soluble CD14 (sCD14), lipopolysaccharide (LPS), LPS binding protein (LBP), IgM endotoxin-core antibody (IgM EndoCAb), and zonulin. BMI, waist circumference and blood pressure were also determined. After 9-week yogurt consumption, YO and YN had decreased TNF-α/sTNFR-RII. Yogurt consumption increased plasma IgM EndoCAb regardless of obesity status. sCD14 was not affected by diet, but LBP/sCD14 was lowered by yogurt consumption in both YN and YO. Yogurt intervention increased plasma 2-arachidonoylglycerol in YO but not YN. YO peripheral blood mononuclear cells expression of NF-κB inhibitor α and transforming growth factor β1 increased relative to CO at 9 weeks. Other biomarkers were unchanged by diet. CO and YO gained approximately 0·9 kg in body weight. YO had 3·6 % lower diastolic blood pressure at week 3. Low-fat yogurt for 9 weeks reduced biomarkers of chronic inflammation and endotoxin exposure in premenopausal women compared with a non-dairy control food. This trial was registered as NCT01686204.

Physical violence is a frequent occurrence in acute community psychiatry units worldwide. Violent acts by patients cause many direct injuries and significantly degrade quality of care. The most accurate tools for predicting near-term violence on acute units rely on current clinical features rather than demographic risk factors. The efficacy of risk assessment strategies to lower incidence of violence on acute units is unknown. A range of behavioral and psychopharmacologic treatments have been shown to reduce violence among psychiatric inpatients.

Background
Subtle formal thought disorders are difficult to quantify. Their relationship to florid thought disorder is unknown.
AimsTo assess the interrater reliability, sensitivity and factor structure of a new assessment instrument, the Thought and Language Index (TLI), and to determine if minor aberrations detectable in the speech of healthy individuals are related to the more severe formal thought disorders characteristic of schizophrenia.
MethodInterrater reliability was evaluated by determining the intraclass correlation for the ratings by five assessors. Factor analysis of the TLI scores of 87 patients was performed, and TLI scores in matched patients and controls were compared.
ResultsThe intraclass correlation was good for individual TLI items, and excellent for sub-scale scores. Factor analysis identified three groups of approximately orthogonal disorders. Mild speech aberrations were observed in healthy participants and in patients with schizophrenia. The prevalence of mild aberrations was correlated with the prevalence of definite formal thought disorders.
ConclusionsThe TLI is reliable and capable of detecting subtle disorders. Some mild aberrations occurring in the speech of healthy individuals appear to be attenuated forms of the florid disorders characteristic of schizophrenia.

Many studies have demonstrated differential resistance to change in the context of negative behavioral contrast. That is, as a result of introducing a disruptor, response rates decrease to a greater extent when the maintaining reinforcement schedule is leaner. Resistance to change also applies to positive contrast, in that increases in response rate are greater in leaner schedules. The negative contrast effects seen in studies of intrinsically motivated behavior reflect an increase in resistance to change as a result of adding extrinsic reinforcers.

The retinas of adult teleost fish can regenerate following injury, but little is known about the neuronal integration of the visual scene that is performed by the regenerated retina. Using goldfish retinal ganglion cells (RGCs) as the experimental system, an evaluation of dendritic arbor structure and passive electrotonic properties was developed, the aim being to quantitatively test the hypothesis that native and regenerated RGC dendritic arbors have similar structural and modeled electrotonic attributes. Fractal dimension was chosen as the descriptor of RGC dendritic arbor complexity, and the arbors' transfer function magnitudes were estimated using an electrically passive, equivalent-circuit analysis. For both native and regenerated RGCs, arbors qualitatively judged to be simple tended to have lower fractal dimension values than arbors judged to be more complex. All cells had similar cut-off frequencies, and for random stimulation of greater than 25% of an RGC's population of dendritic tips, there was a positive correlation between fractal dimension and transfer function magnitude. Some regenerated RGCs had abnormally long primary dendrites, but neither the distributions of fractal dimension values, nor the estimated transfer function magnitudes, were significantly different between native and regenerated RGCs. The results appear to support the hypothesis that structural and modeled electrotonic attributes of regenerated goldfish RGCs are similar to those of native RGCs, suggesting that regenerated RGCs may restore normal visual function.

1. The pool size, space and rate of irreversible loss of glucose were estimated with primed infusions of [U-14C]glucose in reindeer cows within 6 h of being taken from outdoor pens or from free grazing in the field.
2. In conjunction with primed infusions of [U-14C]glucose, single injections of [3-3H]glucose were also used to estimate pool size, space, transfer rate, and turnover time of glucose.
3. Except in a period of severe undernutrition, the concentration of glucose in plasma was higher (range 0·76–1·40 mg/ml) than that recorded for other ruminants.
4. The size of the glucose pool (range 8–35 g) varied in parallel with plasma glucose concentration and was generally distributed in a space in excess of the extracellular fluid volume.
5. The lowest rates of irreversible loss of glucose (approximately 1·7 mg/min per kg0·75) were measured when cows were in mid pregnancy and when available food was scarce; the highest rate (5·5 mg/min per kg0·75) was found in cows during mid summer.
6. Changes in irreversible loss and transfer rate of glucose are interpreted in relation to changes in body composition (estimated in a parallel study), subjective assessment of available food and factors known to control glucose metabolism in other ruminants.
7. The difference between glucose transfer rate and rate of irreversible loss of glucose was used as an index of the rate of resynthesis of glucose from products of glucose catabolism. The rates of glucose resynthesis were highest during a period of rapid growth (4·52 mg/min per kg0·75 or 45% of the glucose transfer rate) and in mid and late pregnancy (respectively 4·14 and 4·28 mg/min per kg0·75 or 71 and 59% of the transfer rate).
